AdventHealth has world-class nutrition programs. Freight Farms, Second Harvest pilots, Food is Health, the 4Roots partnership. What's missing is a shared logistics, data, and operational layer connecting them to the 5% mandate.

FareRx can be that connective tissue. One partner that aggregates local food, delivers it via mobile teaching kitchens, and extends clinical nutrition into the community. Complementary to everything AdventHealth is already doing. Operational from day one.

Our role is to help connect AdventHealth's existing programs and fill the operational gaps between mandate and execution. No competition with 4Roots, Freight Farms, or Second Harvest. No system-wide integration on day one.

Who We Are

Food First. Clinically Driven.

FareRx is a food-as-medicine company based in Philadelphia. We run a physician-supervised, RDN-led cardiometabolic care program built inside a community teaching kitchen — connected by remote patient monitoring and powered by our own grocery fulfillment infrastructure.

We've served over 8,000 members, completed more than 20,000 grocery deliveries, and built the clinical, logistics, and data infrastructure to do this at scale. We're ready to bring that experience to AdventHealth.

We can extend AdventHealth’s clinical reach with our cardiometabolic care model — MNT, DSME, RPM, CCM, and RTM delivered through standard referral pathways. AdventHealth keeps the patient relationship. We help keep them connected to care.
